This form must be completed and signed by the student, and/or parent if the student
is under 19 years of age. The 太阳城集团官方网站 Health and Counseling Form will ask
students to provide proof of the following:

- Date(s) of first and second measles (rubella), mumps rubella, or MMR after the age of twelve months (Not required if born before January 1, 1951).
