Immunizations
PROOF OF MEDICAL IMMUNIZATIONS IS REQUIRED FOR ADMISSION
A Peru State College Health and Counseling Form will be sent to admitted students.
This form must be completed and signed by the student, and/or parent if the student
is under 19 years of age. The Peru State College Health and Counseling Form will ask
students to provide proof of the following:
- A copy of immunizations (your shot record) from M.D.; clinic; baby book; middle or high school records, must be sent, it is mandatory!
- Date(s) of first and second measles (rubella), mumps rubella, or MMR after the age of twelve months (Not required if born before January 1, 1951).
- Last childhood polio vaccine date - last childhood DPT.
- INTERNATIONAL STUDENTS ONLY: Tuberculosis Skin Test. administrated within ONE YEAR before admission to Peru State College.
